academic visitor or tier 5? NHS implications?

academic visitor or tier 5? NHS implications?

I am an Torontonian-American professor, who is a Candadian permanent resident, who has been invited by the Leverhulme Trust and a UK university to spend my sabbatical year there to do some writing and give research presentations. The Trust is reimbursing my living costs while I am there. I am not working for them or the university though.

Which visa would I apply for?

Also, do the two visas have different implications v/v the NHS? I can't live there for without health coverage...

If you're seeking to get free access to NHS, you should apply under Tier 5 as visitors don't have that privilege (unless emergency treatments). Even with Tier 5, some surgeries can refuse you as it's a temporary status, but you definitely have more chances than under a visitor status.
