The US has the highest maternity mortality rate in the developed world! I wouldn't exactly say she'd get better care in the US over Canada. In fact, I'd urge you to return to Canada and have the baby there, you're both likely to survive and have a healthy delivery there! Not to mention of course the cost of healthcare and delivery in the US is just crazy. It's a wonder anyone has babies at all :p

Yes, you will always have to have written permission from the father of the baby if you and the child travel without him. Both countries border patrols require this because of so many non-custodial parents kidnapping their own children and skipping over the border in past years. Regardless of the child's citizenship, he/she will be required to travel with written consent. Just a simple letter hand written by the parent, and notarized usually will suffice.

I'm going to give you my last bit of advice which I hope helps. Discuss your future goals and aspirations with your unborn child's father. This is very important! If you plan on staying in the U.S. have the baby there. If you're intent is to move back to Canada, have the baby in Canada. Do what is comfortable for you and make sure you are well prepared. Healthcare insurance in the U.S. is NOT excellent. I am with a well known insurance company and if I decide to have kids one day, they will only cover half of the doctor's visits and hospital expenses. I will have to foot the rest of the bill myself. So keep that in mind. It sounds as though you've already decided to have your baby in the states so with that said, make the proper preparations that will benefit the needs of your child and get your visa paperwork in order. That is a must. Again I will say good luck with everything.

I think it boils down to whether you have good healthcare coverage or not. If you don't, have it in Canada, if you do, have it in the US. It wasn't that long ago that Calgary Health Region was sending woman having premature births down to Great Falls because they had run out of beds.

It's not to do with citizenship, it's do with criminal law on parents absconding with children. Even if you were all US citizens and you try to cross the border into Canada then they want to see evidence the other parent is okay with that, or a court has granted sole custody. As far as they know you could be kidnapping the child.
