Canadian w/American Pregnant Wife and Child

Hi,

I am a Canadian citizen living in the United States with an American wife who is pregnant. I am contemplating moving back home to Canada and am curious what the best path would be with regards to my wife needing landed immigrant status. Is it best to apply from abroad? If so, how long does it take? If we do it from Canada, will they let her into the country even though she is pregnant? As I understand it, my wife will quality for BC medical coverage a couple of months after she receives her status, which can take up to two years. Is this true? If so, if we apply from inside Canada how will we be medically covered for her pregnancy?

I would appreciate any guidance on this.
