Change of city and business name on a TN

I am on a TN in MA as veterinarian. The clinic where I worked since June had to change buildings and we ended up moving 2 cities away. The owner also wants the name to be changed from Pet Clinic to Veterinary Hospital. My TN is listed under the old name and city. Does anyone know what I need to do to get it updated? Is there a phone number or form that my employer can use so immigration gets the new info?
