I appreciate it if you could give me some advice.
I am a Canadian citizen. When I first applied for a Canadian passport long time ago, there was an option to choose how your name would appear on the passport. For example, you could exclude your middle name, or use your middle initial, or use your middle name (if this is what you have been using as your given name). Since my given name is actually my middle name, I chose to have my passport have my name as "Middle Name" + "Last name". During renewals, I use the short form of the application, which means that the name on the new passport remains the same as the old passport. So my current passport also has my name as "Middle Name" + "Last name". Until now, this has NEVER been an issue - because my given name is on my passport. Note that my other documents (citizenship card, driver's license, etc.) has my full legal name as "First" + "Middle" + "Last".
Now I have an offer from a university in the US. Eventually I will go for the PR status, but initially I will have to apply for H1B visa. During the application process I am not sure how I should fill the forms. They ask First, Middle and Last name. It is just a mess. Should I fill the form as my name appear on my passport, i.e. first name = my given name, and leave the middle name field blank? Or should I provide the legal name as appear in all my other documents. If I do this, how will my I-797 will come back? It will probably come back with my full legal name, which will have an extra field (first name) that my passport does not have. Will this create a problem at the POE?
In addition, will this create problems later in obtaining SSN, driver's license, green card, etc.?
Any advice will be highly appreciated.
