How do I Cancel OHIP (Fax Number?)?

Finding the form 0280 was easy -- forms.ssb.gov.on.ca/mbs/ssb/forms/ssbforms.nsf/AttachDocsPublish/014-0280-82~7/$File/0280-82E_.pdf (add three w's in front of this link, the forums anti-spam software prevents me from linking)

Finding out how to file it is turning out not to be easy. I think I've browsed all over the website, trying phoning in dozens of times only to wait on hold for long periods of time without ever getting answered.

The website lists addresses of OHIP offices to visit, but I've yet to find a fax number.

To whom? Should I just choose a random OHIP office and mail the form + letter to them?

I am quite certain there is a main office that you could mail it to. Its been so long ago that I did it, I don't remember where I sent it. You could call their service number and ask them.
