If you need a passport application form you can dowload it (and print it) from the passport office website :

I went to Invicta Studio ( 83 Oxford Street, Manchester, M1 6EG) for my passport photos (canadian). It costs £12.00 and they do the correct size and specifications (you have to bring the specifications with you ).
