Question about petitioning for a fiance visa

I live in Canada any my fiance lives in the U.S. We want to get a fiance visa and have filled out the petition form. Now I'm wondering what all else we need to send in with the petition. I know it says something about a statement of intent to marry within 90 days of my entry into the U.S. What do we need to write in our statements? I'm also not sure of a sure fire way to prove that we've met within the last 2 years. Any help with these questions would be much appreciated.
