The toatlization agreement addresses your situations, but you generally don't need it.
Whether or not you go back to Canada:
CPP: you qualifify based on working one or more year in canada.
SS: you qualify by having contributed 40 quater years. Even if you had only worked 18 months and gone back to canada you would have gotten some SS.
Medicare: you qualify due to your 40 quarters of SS.
OAS: You will get x/40th of the maximum, based on the number of years you live9d) in canafa between the ages of 18 and 65. You have, say, 8 years now, plus whatever you have when you go back, if you go back. If you do not go back, you will still get 8/40th, but will have to prove you worked in US for 12 years (you need to show 20 years of canad/US life to get OAS). that is the only part of the totalization agreement that would apply to you.
